Home-Based Care

Care Management

 

Note: Care Management

These programs develop plans for coordinated care of individuals who, because of age, illness, disability, or other difficulties are unable to arrange for services on their own behalf.  Services include assessment of needs, coordination of needed services, and monitoring/follow-up of services to determine their effectiveness. 

 

Only non-profit care management agencies are listed below. The National Association of Professional Geriatric Care Managers provides a registry of private pay care managers with a specialized focus on issues related to aging and elder care at www.caremanager.org. 

 

Community Care Management Corp.

Multi-program, private non-profit agency that helps people in Lake and Mendocino counties to live independently in their own home settings. See www.communitycare707.com. Programs include:

 

·        Community Care HIV/AIDS Program (CCHAP)

CCHAP provides care management to persons in Lake and Mendocino Counties who have been diagnosed with HIV or AIDS.

 

·        Linkages Program

Linkages care management serves adults (age 18+) with disabilities who require some assistance to live independently. Linkages cannot serve persons who are eligible for Regional Center, Mental Health, or Department of Rehabilitation case management services.

 

·        Multipurpose Senior Services Program (MSSP)

MSSP provides social and health care management to assist persons age 65+ to remain safely at home. Clients must be Medi-Cal eligible without a share of cost, and at risk for institutionalization.

 

Mendocino County AIDS Volunteer Network (MCAVN)

Case management, practical assistance, emotional support, drop-in center, referral and information, food vouchers, transportation, rent and utilities assistance. HIV/HCV counseling and testing, syringe exchange, HCV support groups, client socials. For any person affected by, living with or at risk for HIV/HCV in Mendocino County. See www.mcavn.org.

 

 

Caregiver Respite & Support

 

Note: Respite Services for Caregivers

Provides supervision and care of clients while the usual caregiver receives a break.

Redwood Caregiver Resource Center (RCRC)

Redwood Coast Medical Services - Shamli Volunteer Hospice/Respite Program

Home visits by medical professionals (such as registered nurses, physical/occupational/speech therapists) and social workers for those unable to leave home to receive these services.  Prescribed

by a doctor and paid by medical insurance subject to policy limitations.

Eligibility: Medi-Cal eligible, 65+ or legally blind or disabled by Social Security standards, disabled children.
